Flying an airliner is a complex task. It requires highly trained pilots working together to get hundreds of tonnes of metal and people safely into the air. The margin for error is small, and when mistakes do happen, they can be catastrophic.
When things go well, this complexity looks like clockwork - each of the components interlinking seamlessly, and ticking away the flight.
The story of Korean Air flight 8509 is a dark and complex one. Its cogs and wheels are psychological, technological, and even cultural. When the clockwork of this flight burst open just seconds after takeoff on December 22nd 1999, it revealed deep rooted problems in the aviation industry - problems which would need to be reckoned with, fast.

Another factor not mentioned there is that with the plane taxiing so slowly, the pilots developed an inaccurate sense of time. They were used to taxiing faster so that all that extra time spent taxiing so slowly gave them a false sense of "We've been taxiing for so long that we must be at the far end of the airport by now." This reinforced their confirmation bias.
They should have never trusted the altimeter, since that was wrong from the beginning. I don't know why they started to distrust everything else, like navigation and the engine power. They had the speed brakes on, and the engines at low power, for quite a while. Why? Also, if the speed brakes were on, and the engines were at low, the only way additional speed could be generated was by descending. They could even have run some simple tests to verify the state of specific intruments. Try to increase speed and see if any reported speed increases. Try to climb and see if any altimeter increases normally. That should have shown that only the air traffic control's speed was correct.
This is your pilot speaking. We hope to land in Islamabad, but I can't be sure because I really don't know how to fly this plane. My bad, should have let you know.

I’m from Perth and this really freaked me out, knowing as I watched this unfold on tv, the window behind my tv looking out into the Indian Ocean, in all that blue darkness, the plane was out there somewhere.
Tragic event. was flying to the USA once, and when we was near LA we where to high in the sky. I dont know if this is possible, but we had a little free fall straight down, all of passangers screamed, even it was controlled. cant imagine the people onboard this plane was feeling

Why do these aviation disaster channels always reveal the end at the very beginning and even in the title?
Given that highly experienced pilots sometimes make critical mistakes, imagine how flying will be when the priority at all the airlines for selection of their flight crews is based on skin color and ficticios gender (DEI) and not on skills and experience.
That's what I call a new style of arm / wrestling.
I like that not all of these stories end with fatalities. I like the videos when the pilots, against all odds, pulls it off, and lands safely

Cannot imagine the fear of passengers flying upside down!!!! The Capt and FO were so awesome in trying their best. How Alaska Air handled their maint checks is abominable. Executives should randomly have to fly as a passenger or better yet in the cockpit and see the actual procedures. I bet tragedies like this one wouldn’t happen!! Very sad and RIP all!!🙏🏻

This horrible unnecessary tragedy plays out like an original "Twilight Zone" episode of the 1960's. And is a testament to your filmmaking prowess. Yeah there needs to be some type of third door code initiation for situations just like this... That poor co-pilot there was nothing he could do but pound on the door.. heartbreaking 😭
He absolutely did it because all of the flight commands were instituted by a person in the cockpit directing them and making them.. these systems didn't just turn themselves on and off and turn the plane left and right up and down in altitude. it's the horrible truth...
The flight is also of note in that the accident drew undesired attention to the Mount Weather facility, which was the linchpin of plans implemented by the federal government to ensure continuity in the event of a nuclear war. The crash did not damage the facility, since most of its features were underground. Only its underground main phone line was severed, with service to the complex being restored by C&P Telephone (Bell Atlantic/Verizon 1984-present) within 2+1⁄2 hours after the crash.
